Background:

Language barriers pose significant challenges in primary care settings.
The Office of Minority Health established Culturally and Linguistically Appropriate Services (CLAS) standards.
Limited research exists on linguistic services in ambulatory care compared to emergency rooms.

Purpose of the Study:

To assess linguistic services in ambulatory care centers against national CLAS standards.
To determine the availability, need, and utilization of interpreter services for patients with limited English proficiency.
To identify gaps in culturally competent care related to linguistic services.

Main Methods:

Review of literature on language barriers in primary care.
On-site visits and survey data collection at eight ambulatory care centers in New England.
Analysis of services based on established CLAS standards.

Main Results:

Most surveyed clinics served predominantly Spanish-speaking patients.
Informal interpreter services were common, but formal, integrated services were rare.
Few clinics fully complied with national CLAS standards for linguistic services.

Conclusions:

Ambulatory care settings face challenges in providing adequate linguistic services.
There is a need for clearer national regulations and dedicated funding for interpreter services.
Integrating linguistic services is crucial for achieving culturally competent primary care.
