A painful mid-hypogastric tumor in a 9-year-old boy was surgically removed. Histology revealed it was an inflammatory urachal remnant, not sarcoma, highlighting the need for surgical resection of suprapubic masses.
Area of Science:
- Pediatric Surgery
- Uropathology
- Surgical Oncology
Background:
- Mid-hypogastric tumors in children can present as painful, prominent, and solid masses.
- Differential diagnosis for such masses includes soft tissue sarcomas and urachal anomalies.
Observation:
- A 9-year-old boy presented with a clinically isolated, mid-hypogastric tumor.
- The tumor was initially suspected to be a sarcoma due to its physical characteristics.
Findings:
- Surgical en bloc resection of the tumor, including the attached vesical cupula, was performed.
- Histopathological examination confirmed the tumor was purely inflammatory, originating from urachal remnants.
- The diagnosis of inflammatory urachal remnants must be considered in cases of suprapubic tumors.
Implications:
- This case underscores the importance of considering urachal remnants in the differential diagnosis of suprapubic masses in pediatric patients.
- Given the potential for malignant transformation of urachal cysts, surgical resection is recommended for suspected urachal anomalies.
- Early and accurate diagnosis can prevent misclassification and ensure appropriate management, avoiding unnecessary aggressive treatments like sarcoma therapy.
