Background:

  • Malignant external otitis (MEO) is a severe infection of the external ear canal.
  • Temporomandibular joint (TMJ) involvement is a rare but serious complication of MEO.

Observation:

  • This study reviewed 6 patients with MEO and TMJ involvement over 8 years.
  • Patients often had type 2 diabetes and presented with otalgia, otorrhea, and granulation tissue.
  • TMJ symptoms, including swelling and trismus, developed months after initial MEO diagnosis.

Findings:

  • TMJ involvement occurred in 14% of MEO patients, associated with resistant infections and recurrences.
  • Common pathogens included Pseudomonas aeruginosa and Staphylococcus epidermidis.
  • Mortality was 50% in this small cohort, highlighting disease severity.
