Summary

Central nervous system infections are serious. Streptococcus pneumoniae is the most common cause of purulent meningitis in adults, while gram-negative bacilli are frequent after trauma or neurosurgery.

Background:

  • Central nervous system (CNS) infections, including meningitis and encephalitis, represent significant medical challenges.
  • Purulent meningitis and intracranial suppurative infections are common and serious conditions requiring prompt diagnosis and treatment.

Observation:

  • A review of 100 adult purulent meningitis cases identified Streptococcus pneumoniae as the predominant pathogen (56%), followed by Neisseria meningitidis (16%) and Listeria monocytogenes (7%).
  • In cases of purulent meningitis following trauma or neurosurgery (25 cases), gram-negative bacilli (14), Staphylococcus aureus (5), and Staphylococcus epidermidis (4) were the most common pathogens.
  • Intracranial suppurative infections show progress in diagnostic and therapeutic modalities, including microbiology, antibiotics, and imaging.

Findings:

  • Streptococcus pneumoniae is the leading cause of adult purulent meningitis.
  • Gram-negative bacilli are a primary concern in post-traumatic or post-neurosurgical meningitis.
  • Subdural empyema remains diagnostically challenging due to its underlying pathology.

Implications:

  • Advances in medical science have improved outcomes for CNS infections.
  • Continued research is needed to address diagnostic difficulties, particularly in cases like subdural empyema.
  • Understanding causative pathogens is crucial for effective treatment strategies in meningitis and encephalitis.
