Background:

  • Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal neoplasms of the gastrointestinal tract.
  • Leiomyomas are benign smooth muscle tumors that can occur anywhere in the gastrointestinal tract, including the stomach.

Observation:

  • A 58-year-old male presented with a 6-month history of an upper abdominal mass, weight loss, and malaise.
  • Abdominal ultrasonography revealed a 10 cm x 7 cm hypoechoic mass with cystic areas anterior to the stomach.
  • Endoscopy showed extrinsic compression of the anterior stomach wall; CT scan confirmed a well-defined, heterogeneous mass with cystic areas attached to the stomach by a stalk.

Findings:

  • Surgical resection of the mass, a portion of the stomach wall, and omentum was performed.
  • Histopathological examination confirmed the mass as a gastric leiomyoma.
  • No tumor deposits were found in the sampled lymph nodes.

Implications:

  • This case highlights the importance of comprehensive imaging and histopathological analysis for diagnosing gastric leiomyomas.
  • Successful surgical management led to a positive long-term outcome for the patient.
  • Early detection and surgical intervention are crucial for managing symptomatic gastric leiomyomas.
