Summary

Low-dose hormone replacement therapy (HRT) offers effective symptom relief and bone protection for menopausal women. This regimen, particularly continuous combined HRT, achieves amenorrhea and endometrial protection with fewer side effects.

Area of Science:

  • Gynecology
  • Endocrinology
  • Menopause Management

Background:

  • Hormone replacement therapy (HRT) use has risen, with a preference for formulations avoiding bleeding disturbances.
  • Continuous combined HRT is favored for achieving amenorrhea.
  • Historically, higher doses of hormones were prescribed for climacteric symptoms.

Purpose of the Study:

  • To evaluate the efficacy and safety of low-dose HRT formulations.
  • To determine if low-dose HRT can effectively manage menopausal symptoms and prevent bone loss.
  • To assess the impact of low-dose continuous combined HRT on amenorrhea and endometrial health.

Main Methods:

  • Review of studies using low-dose estrogens (e.g., estradiol 1 mg, conjugated equine estrogens 0.3 mg, transdermal estradiol 25 μg) and progestogens.

Related Experiment Videos

  • Analysis of HRT regimens focusing on continuous combined administration.
  • Assessment of outcomes including vasomotor symptom control, bone mineral density, amenorrhea rates, endometrial protection, lipid profiles, and side effects like mastalgia.
  • Main Results:

    • Low-dose HRT effectively treats vasomotor symptoms and prevents bone loss.
    • Continuous combined low-dose HRT achieves amenorrhea in most women within months.
    • Continuous low-dose progestogen protects the endometrium from hyperplasia.
    • Favorable effects on lipid and lipoprotein patterns were observed.
    • Fewer side effects, such as mastalgia, were reported compared to high-dose regimens.

    Conclusions:

    • Low-dose HRT is a rational and effective alternative for menopausal women.
    • These lower doses are sufficient for the majority of women seeking HRT.
    • Continuous combined low-dose HRT provides symptom relief, bone protection, and endometrial safety with improved tolerability.
