Summary

A man with persistent acrodermatitis continua experienced widespread pustules resistant to methotrexate. Low-dose cyclosporine effectively treated this severe skin condition, inducing lasting remission.

Background:

  • Acrodermatitis continua is a rare, chronic pustular dermatosis of the extremities.
  • It is often recalcitrant to conventional therapies, leading to significant morbidity.

Observation:

  • A 69-year-old male presented with painful, localized acrodermatitis continua.
  • The condition progressed to widespread pustulation despite high-dose methotrexate treatment.

Findings:

  • Low-dose cyclosporine therapy was initiated for the widespread pustular eruption.
  • Cyclosporine induced a rapid and sustained remission of the acrodermatitis continua.

Implications:

  • Low-dose cyclosporine represents a viable therapeutic option for severe, methotrexate-resistant acrodermatitis continua.
