Lung cancer (Amsterdam, Netherlands)·2024

Area of Science:

  • Medical diagnosis
  • Thoracic surgery
  • Infectious diseases

Background:

  • Pancoast tumors, often associated with lung cancer, can present with shoulder and arm pain.
  • Differential diagnosis for Pancoast tumor symptoms includes various thoracic conditions.

Observation:

  • A 48-year-old man presented with left shoulder pain, radiating to the scapula, and left arm tingling.
  • Initial clinical and radiological findings suggested a Pancoast tumor of the left lung.

Findings:

  • Computed tomographic guided fine needle biopsy was inconclusive.
  • Video-assisted thoracoscopic surgery enabled a conclusive biopsy.
  • Histologic and microbiologic examinations confirmed tuberculosis (TB) as the diagnosis.

Related Experiment Videos

Implications:

  • This case highlights the importance of considering tuberculosis in the differential diagnosis of Pancoast tumor presentations.
  • Thoracoscopic biopsy can be crucial for definitive diagnosis when less invasive methods fail.
  • Accurate diagnosis is essential for appropriate treatment of thoracic conditions presenting with Pancoast tumor-like symptoms.
