Summary

This study reports the first case of lupus nephritis featuring unique crystalline structures within the kidneys. These IgG and kappa light chain crystals offer new insights into lupus nephritis pathogenesis.

Background:

  • Systemic Lupus Erythematosus (SLE) frequently involves the kidneys, leading to lupus nephritis.

Observation:

  • A renal biopsy from a 30-year-old woman with SLE revealed electron-dense deposits in the glomeruli.
  • Remarkably, these deposits and surrounding mesangial areas contained electron-dense crystalline material.

Findings:

  • Immunoelectron microscopy identified the crystalline structures as composed of IgG and kappa light chains.
  • Cryoglobulinemia and paraproteinemia were notably absent, suggesting alternative formation pathways.
