Background:

  • Gaucher's disease is characterized by lysosomal accumulation of glucocerebroside.
  • Miglustat is an oral substrate reduction therapy targeting ceramide glucosyltransferase.
  • This inhibition aims to prevent glucocerebroside accumulation in Gaucher's disease.

Purpose of the Study:

  • To evaluate the efficacy of miglustat in reducing organ volumes in type 1 Gaucher's disease.
  • To assess the impact of miglustat on hematological parameters.
  • To compare miglustat combined with enzyme replacement therapy (ERT) versus ERT alone.

Main Methods:

  • Noncomparative trials and a randomized study were conducted.
  • Patients received miglustat at 50 or 100mg three times daily for 6-12 months.
  • A subset of patients received miglustat plus ERT compared to ERT alone.

Main Results:

  • Miglustat significantly reduced liver and spleen volumes, with greater reduction at higher doses.
  • Platelet counts improved significantly at 12 months with the higher miglustat dose.
  • Miglustat plus ERT showed greater liver volume reduction than ERT alone.

Conclusions:

  • Miglustat demonstrates efficacy in reducing organomegaly in type 1 Gaucher's disease.
  • Combination therapy with ERT may enhance therapeutic benefits.
  • Diarrhea and weight loss are common adverse events; fine tremor is also reported.
