Summary

Cardiac glycoside intoxication, though common, rarely causes nonocclusive mesenteric ischemia. This rare complication, seen in a digitoxin overdose case, can lead to ischemic colitis.

Area of Science:

  • Cardiology
  • Gastroenterology
  • Toxicology

Background:

  • Nonocclusive mesenteric ischemia (NOMI) is a severe condition with high mortality.
  • Cardiac glycosides like digoxin and digitoxin are potential causes, alongside hypotension, heart failure, and septic shock.

Observation:

  • A case of digitoxin intoxication is presented.
  • The patient developed autopsy-confirmed ischemic gangrenous colitis.

Findings:

  • Intoxication with cardiac glycosides is frequent.
  • NOMI is an infrequent complication of such intoxication.

Implications:

  • NOMI should be considered in patients on cardiac glycosides presenting with diffuse abdominal pain.
  • Early recognition and management are crucial for improving patient outcomes.
