Summary

Photodynamic therapy (PDT) for pathologic myopia-related choroidal neovascularization (CNV) can lead to lacquer crack formation. This complication, characterized by subretinal hemorrhage, occurred despite minimal chorioretinal damage from PDT.

Background:

  • Pathologic myopia is a leading cause of choroidal neovascularization (CNV).
  • Photodynamic therapy (PDT) using verteporfin is a treatment for CNV.
  • Lacquer cracks are a known complication in pathologic myopia.

Observation:

  • A 42-year-old woman with pathologic myopia underwent PDT for juxtafoveal CNV.
  • Pre-treatment angiography showed no lacquer cracks.
  • Post-PDT, a large lacquer crack with subretinal hemorrhage developed in the treated macula.

Findings:

  • PDT effectively subsided the CNV.
  • A significant lacquer crack formed shortly after PDT.
  • The lacquer crack was located beneath a subretinal hemorrhage.

Implications:

  • Even minimal chorioretinal damage from PDT can predispose pathologic myopia eyes to lacquer crack formation.
  • This highlights a potential risk associated with PDT in this patient population.
  • Further research may be needed to understand and mitigate this complication.
