Summary

Postherpetic itch (PHI) is a common complication of shingles (herpes zoster), often accompanying or replacing pain. Research is needed to understand its frequency, impact, and effective treatments.

Area of Science:

  • Neurology
  • Dermatology
  • Pain Medicine

Background:

  • Postherpetic neuralgia (PHN) is a well-known complication of shingles (herpes zoster).
  • Neuropathic itch, termed postherpetic itch (PHI), is often experienced by patients with shingles, sometimes severely.
  • PHI is underrepresented in medical literature, lacking epidemiological and treatment studies.

Purpose of the Study:

  • To investigate the epidemiology of pruritus (itch) during and after shingles.
  • To analyze existing data to understand the occurrence and characteristics of PHI.

Main Methods:

  • Analysis of 3 independent datasets comprising 586 adults with shingles or PHN.
  • Data focused on itch localized to shingles-affected areas.

Related Experiment Videos

Main Results:

  • Pruritus commonly accompanies acute zoster and PHN, typically mild to moderate.
  • No significant age difference was found between subjects with and without PHI.
  • Shingles affecting the head, face, and neck were associated with a higher likelihood of PHI compared to torso involvement.

Conclusions:

  • Postherpetic itch is a common, yet understudied, complication of shingles.
  • Further research, including prospective studies, is necessary to determine PHI's frequency, impact, and optimal treatments.
