Summary

Transfusion-associated graft-versus-host disease (TA-GVHD) is a fatal complication of blood transfusions. Gamma-irradiation of blood components effectively prevents TA-GVHD in at-risk patients.

Area of Science:

  • Hematology
  • Immunology
  • Transfusion Medicine

Background:

  • Transfusion-associated graft-versus-host disease (TA-GVHD) is a severe complication of blood product transfusion.
  • It results from donor T lymphocytes attacking recipient tissues, often leading to fatal outcomes.
  • High-risk patient groups include those with immune defects or receiving directed donations.

Purpose of the Study:

  • To highlight the risks and clinical presentation of TA-GVHD.
  • To emphasize the critical need for preventative strategies against TA-GVHD.
  • To recommend effective methods for preventing TA-GVHD.

Main Methods:

  • Review of clinical presentation and risk factors for TA-GVHD.
  • Evaluation of preventative measures for TA-GVHD.
  • Assessment of the efficacy of gamma-irradiation in TA-GVHD prevention.

Main Results:

  • TA-GVHD diagnosis is considered in patients with fever, rash, and potential GI symptoms or pancytopenia post-transfusion.
  • Mortality rates for TA-GVHD exceed 90% with current treatments.
  • Pretransfusion gamma-irradiation at 25 Gy effectively prevents TA-GVHD.

Conclusions:

  • TA-GVHD is a life-threatening transfusion complication requiring stringent preventative measures.
  • Gamma-irradiation of blood components is a highly effective prophylactic method.
  • Irradiation should be standard for blood components transfused to at-risk patient populations.
