Background:

Epidermoid cysts of the testis are uncommon in pediatric patients, representing only 3% of all testicular tumors.
Bilateral epidermoid testicular cysts are exceptionally rare in children, with only two prior documented cases.
Association with genetic conditions like Klinefelter's syndrome has not been previously reported.

Observation:

The case involves a young boy diagnosed with bilateral epidermoid testicular cysts.
The patient also presented with Klinefelter's syndrome, a novel association in pediatric cases.
Clinical presentation and diagnostic imaging were consistent with epidermoid cysts.

Findings:

This is the first reported instance of bilateral epidermoid testicular cysts co-occurring with Klinefelter's syndrome in a pediatric patient.
The study highlights the unique clinical presentation and diagnostic challenges.
Management and therapeutic strategies were evaluated for this specific case.

Implications:

This case expands the understanding of epidermoid cyst presentation in children, particularly in the context of genetic syndromes.
