Summary

Radiation therapy can cause long-term heart damage, including valve dysfunction and artery blockages. Regular cardiac evaluations are crucial for early detection and intervention in survivors.

Area of Science:

  • Cardiology
  • Oncology
  • Radiology

Background:

  • Radiation therapy, commonly used for breast cancer, can lead to chronic cardiac damage years after treatment.
  • Cardiac structures like heart valves and arteries are susceptible to radiation-induced injury.

Observation:

  • A 72-year-old woman, treated for breast cancer 25 years prior, presented with tachycardia and acute dyspnea.
  • Imaging revealed valvular thickening, moderate to severe valvular dysfunction, and significant occlusive coronary artery disease.

Findings:

  • Radiation exposure resulted in severe valvular dysfunction and coronary artery disease.
  • The left subclavian artery was also affected, necessitating immediate surgical intervention.

Implications:

  • Long-term cardiac surveillance is essential for radiation-exposed patients, even if asymptomatic.
  • Early intervention with valvular replacement and myocardial revascularization can improve outcomes.
  • Restrictive myopathy and chronic pericarditis are potential complications that require careful assessment.
