Summary

Continuous alternating-side myoclonic status epilepticus (SE) occurred in an adult after brain injury. This rare presentation in adults, typically seen in newborns, may stem from diffuse cerebral anoxia and commissural fiber damage.

Area of Science:

  • Neurology
  • Neuroscience
  • Critical Care Medicine

Background:

  • Posttraumatic anoxic coma can result from severe brain injury.
  • Myoclonic status epilepticus (SE) is a rare neurological emergency.
  • SE is typically observed in neonatal populations.

Observation:

  • A 66-year-old woman presented with continuous alternating-side myoclonic jerks.
  • The patient experienced diffuse cerebral fat embolism leading to anoxic coma.
  • This clinical presentation is highly unusual for adult patients.

Findings:

  • The patient exhibited myoclonic status epilepticus (SE) with lateralized features.
  • The unusual SE presentation in an adult was linked to diffuse cerebral anoxia.
  • Commissural fiber damage was postulated as a contributing factor.

Related Experiment Videos

Implications:

  • This case highlights the potential for SE in adult anoxic brain injury.
  • Understanding the pathophysiology of adult SE can inform treatment strategies.
  • Further research is needed to explore the mechanisms of SE in diffuse brain injury.
