Summary

Intractable ulcerative colitis (UC) patients may have more carcinoids than previously believed. These neuroendocrine tumors might be a reactive response to chronic inflammation in UC.

Background:

  • Ulcerative colitis (UC) is a chronic inflammatory bowel disease affecting the colon.
  • Long-standing UC increases colorectal cancer risk, necessitating surveillance for dysplasia.
  • Surgical intervention like total proctocolectomy is considered for intractable disease.

Observation:

  • A male patient with a 17-year history of extensive ulcerative colitis underwent total proctocolectomy.
  • Preoperative colonoscopy did not reveal any signs of dysplasia.
  • Histological examination of the resected specimen identified 21 microcarcinoid tumors.

Findings:

  • Microcarcinoid tumors were found within the muscularis propria and superficial submucosa.
  • These findings were in addition to chronic inflammatory infiltrates characteristic of UC.

  • No dysplasia was identified in the resected colorectum.
  • Implications:

    • Carcinoid tumors may be more prevalent in ulcerative colitis patients than currently recognized.
    • The presence of microcarcinoids could represent a reactive phenomenon secondary to chronic inflammation.
    • Further research is warranted to understand the etiology and clinical significance of carcinoids in UC.
