Background:

  • A patient with a history of multinodular goiter and hyperthyroidism, previously treated effectively, developed essential cryoglobulinemia.
  • Interferon-alpha therapy was initiated for cryoglobulinemia in a clinically euthyroid patient with no initial thyroid autoantibodies.

Observation:

  • Following a year of interferon-alpha treatment, the patient exhibited goiter enlargement and laboratory results showing apparently elevated free T3 and T4 levels.
  • Despite elevated thyroid hormone levels, the patient remained clinically euthyroid with normal TSH (thyroid-stimulating hormone) levels.

Findings:

  • High serum levels of antithyroid hormone antibodies were detected using a radiolabeled hormone immunoprecipitation assay.
  • Elevated titers of antithyroglobulin and antimicrosome antibodies were also observed, paralleling the antithyroid hormone antibodies.
