Background:

  • Neurological sarcoidosis is a rare granulomatous disease affecting the central nervous system.
  • Accurate diagnosis and characterization of neurosarcoidosis lesions are crucial for patient management.

Observation:

  • MRI, including T1- and T2-weighted spin-echo sequences, was performed on nine patients with neurological sarcoidosis.
  • Gadolinium contrast enhancement was utilized in seven patients to further evaluate lesions.

Findings:

  • Non-specific lesions were identified, most commonly as high-intensity signals on T2-weighted sequences in the periventricular white matter and semi-oval centers.
  • Other observed abnormalities included hypothalamo-pituitary region infiltrations, granulomatous brain masses, and meningeal infiltrations.
  • Gadolinium-enhanced MRI demonstrated lesions not apparent on conventional sequences, highlighting its importance.

Implications:

  • MRI with gadolinium contrast is valuable for detecting and characterizing neurosarcoidosis lesions.
  • These findings contribute to understanding the imaging spectrum of neurological sarcoidosis.
  • Improved lesion detection may aid in diagnosis and treatment monitoring for neurosarcoidosis.
