Summary

Chronic manganese exposure causes parkinsonism, but not primarily by damaging dopamine transporters. (99m)Tc-TRODAT-1 SPECT imaging helps distinguish manganism from Parkinson's disease.

Area of Science:

  • Neuroscience
  • Toxicology
  • Radiology

Background:

  • Chronic manganese exposure can lead to parkinsonism with clinical features distinct from idiopathic Parkinson's disease (PD).
  • The precise mechanisms underlying manganese-induced parkinsonism are not fully understood.
  • Dopamine transporter (DAT) imaging using (99m)Tc-TRODAT-1 reflects presynaptic dopaminergic terminal function.

Purpose of the Study:

  • To evaluate dopamine transporter (DAT) function using (99m)Tc-TRODAT-1 SPECT.
  • To investigate the integrity of presynaptic dopaminergic terminals in patients with manganese-induced parkinsonism.
  • To differentiate chronic manganism from Parkinson's disease using DAT imaging.

Main Methods:

  • Brain (99m)Tc-TRODAT-1 SPECT imaging was performed on 4 patients with chronic manganese intoxication.

Related Experiment Videos

  • Clinical data and Unified Parkinson's Disease Rating Scale scores were collected for manganism patients.
  • Control groups included 12 patients with PD and 12 healthy volunteers.
  • Main Results:

    • Manganism patients exhibited a bradykinetic-rigid syndrome.
    • (99m)Tc-TRODAT-1 uptake values in the corpus striatum of manganism patients were significantly higher than in PD patients but slightly lower than in healthy controls.
    • A mild decrease in (99m)Tc-TRODAT-1 uptake was observed in the putamen of manganism patients compared to controls, suggesting presynaptic dopaminergic terminals are not the primary target.

    Conclusions:

    • Presynaptic dopaminergic terminals are not the main target of chronic manganese intoxication.
    • (99m)Tc-TRODAT-1 SPECT is a valuable tool for differentiating chronic manganism from Parkinson's disease.
    • This imaging technique offers a convenient and cost-effective method for differential diagnosis.
