Summary

Stroke Units (SUs) significantly reduce in-hospital case fatality and length of hospital stay compared to neurological wards (NWs). These benefits are reproducible in routine practice, especially for older patients and those with severe strokes.

Purpose of the Study:

  • To compare the outcomes of patients treated in a newly implemented Stroke Unit (SU) versus a neurological ward (NW).
  • To assess if the beneficial effects of SUs observed in trials are reproducible in a real-world setting.

Main Methods:

  • A before-and-after comparison of 352 patients in SUs and 352 patients in NWs.
  • Multivariate analysis was used to adjust for case-mix differences.
  • Key outcome measures included in-hospital case-fatality, patient independence at discharge, length of hospital stay (LOHS), and medical complication rates.
