Summary

Lateral pelvic lymph node dissection (LPLD) may improve survival for lower rectal cancer patients with N1 lymph node metastasis. Further randomized trials are needed to confirm the benefits of LPLD in these cases.

Area of Science:

  • Oncology
  • Surgical Oncology
  • Rectal Cancer Research

Background:

  • Investigating oncological outcomes for lower rectal cancer patients undergoing curative surgery.
  • Assessing the potential benefit of lateral pelvic lymph node dissection (LPLD).

Purpose of the Study:

  • To determine if LPLD improves outcomes in patients with lower rectal cancer.
  • To analyze survival rates based on LPLD status in stage II and III lower rectal cancer.

Main Methods:

  • Retrospective review of 246 patients with stage II and III lower rectal cancer (1985-1998).
  • Comparison of outcomes between patients who underwent LPLD and those who did not (42 patients).

Main Results:

  • No survival difference between groups for overall stage II/III disease.

Related Experiment Videos

  • Significant improvement in 5-year disease-free survival for N1 patients with LPLD (73.3% vs. 35.3%, P=0.013).
  • LPLD identified as a significant prognostic factor via multivariate analysis.
  • Conclusions:

    • LPLD appears to enhance prognosis for stage III lower rectal cancer with limited lymph node metastasis.
    • A randomized clinical trial is recommended to validate the efficacy of LPLD.
