Aortic dissection can cause severe heart failure. A complex aortic dissection involving the aortic root and arch was successfully treated with valve-sparing surgery and aortic arch replacement, with no complications.

Background:

  • Acute aortic regurgitation and congestive heart failure can be life-threatening complications of aortic dissection.
  • Stanford type A aortic dissection involving the aortic root and arch presents significant surgical challenges.

Observation:

  • A 61-year-old male presented with chest pain and acute heart failure secondary to massive acute aortic regurgitation.
  • Computed tomography revealed Stanford type A aortic dissection extending from the aortic root to the common iliac arteries.

Findings:

  • The patient underwent David's aortic valve-sparing operation and total aortic arch replacement due to extensive dissection involving aortic commissures and arch vessels.
  • The surgical procedure was well tolerated, with no significant intraoperative or postoperative complications.

Implications:

  • Valve-sparing aortic root replacement combined with total aortic arch replacement is a viable and safe surgical option for complex Stanford type A aortic dissections.
  • This approach can effectively manage aortic regurgitation and prevent further cardiovascular complications in patients with extensive aortic pathology.
