Background:

  • A bufenolide-structure inhibitor of the sodium pump (Na/K ATP-ase) was previously isolated from human placentas.
  • This endogenous compound has a molecular weight of 370 Da.

Observation:

  • The study investigated the presence of this sodium pump inhibitor in non-pregnant individuals, specifically in plasma ultrafiltrate from clinically volume-expanded patients.
  • Five volume-expanded patients (one on haemofiltration, four on plasma exchange) were analyzed.

Findings:

  • HPLC-purified fractions from all five volume-expanded patients inhibited the human leucocyte sodium pump in vitro.
  • These fractions contained a 370 Da compound, identical in mass to the placental inhibitor.
  • However, only one sample showed a mass spectrometry fragmentation pattern matching the placental inhibitor; the other four exhibited a different pattern, suggesting an isomer.
