Background:

  • Bone marrow comprises red (hematopoietic) and yellow (fatty) marrow.
  • Yellow marrow can convert to red marrow, a process termed marrow reconversion.
  • Magnetic resonance imaging (MRI) can be misleading in diagnosing bone marrow neoplastic processes.

Observation:

  • A patient with non-Hodgkin's lymphoma presented with vertebral invasion.
  • Clinical symptoms mimicked inflammatory myositis.
  • MRI of thigh muscles revealed femoral nodular lesions.

Findings:

  • The observed femoral nodular lesions were suggestive of bone marrow reconversion.
  • Bone marrow reconversion is a physiological and reversible process.
  • Radiographic features of bone marrow reconversion can be mistaken for neoplastic infiltration.
