In heart failure patients with coronary artery disease, an atherogenic lipid profile promotes inflammation. This association was not observed in patients with idiopathic dilated cardiomyopathy, suggesting different disease mechanisms.
Area of Science:
- Cardiology
- Biochemistry
- Immunology
Background:
- Low total cholesterol has been linked to poor survival in chronic heart failure, challenging lipid-lowering therapy benefits.
- Inflammation plays a key role in heart failure pathogenesis.
Purpose of the Study:
- To investigate the relationship between lipoprotein concentrations and inflammatory markers (IL-6, TNF-alpha, sTNF-alpha receptors).
- To differentiate these relationships in heart failure patients with coronary artery disease (CAD) versus idiopathic dilated cardiomyopathy (IDCM).
Main Methods:
- Seventy-one patients with chronic heart failure (36 CAD, 35 IDCM) were enrolled.
- Plasma levels of lipoproteins, IL-6, TNF-alpha, and soluble TNF-alpha receptors (sTNFR1, sTNFR2) were measured.
Main Results:
- In the overall cohort, cholesterol/HDL ratio correlated with TNF-alpha, sTNFR1, and sTNFR2.
- In CAD patients, cholesterol/HDL and triglycerides correlated with sTNFR2.
- No significant correlations were found between lipoproteins and inflammatory markers in IDCM patients.
Conclusions:
- An atherogenic lipid profile is associated with increased inflammation in heart failure due to CAD.
- This link between lipids and inflammation is absent in heart failure due to IDCM, indicating distinct pathophysiological pathways.
