Area of Science:

  • Dermatology
  • Allergology

Background:

  • Palmar and plantar psoriasis presents unique challenges in diagnosis and management.
  • Contact sensitization is a potential, yet understudied, factor in psoriasis etiology.

Purpose of the Study:

  • To determine the incidence of contact sensitization in patients with palmar and plantar psoriasis.
  • To evaluate the significance of contact allergy as a causative factor in these specific psoriasis presentations.

Main Methods:

  • A two-year study involving 48 patients with palmar/plantar psoriasis and 61 controls without these lesions.
  • Utilized patch testing to identify contact allergens in the study population.

Main Results:

  • Positive patch tests were observed in 41.7% of patients with palmar-plantar psoriasis.
  • Only 6.6% of psoriatic patients without palmar-plantar involvement showed positive patch tests.
  • Nickel sulfate, mercapto mix, balsam of Peru, potassium dichromate, mercury mix, and fragrance mix were the most frequent allergens.

Conclusions:

  • There is an increased incidence of contact allergy in patients diagnosed with palmar-plantar psoriasis.
  • Contact sensitization may play a significant role in the development or exacerbation of palmar and plantar psoriasis.
