Summary

Hypersensitivity myocarditis, a drug reaction affecting the heart, can occur in heart transplant candidates. This case shows complete reversal of hypersensitivity myocarditis after left ventricular assist device implantation and cessation of inotropic therapy.

Background:

  • Hypersensitivity myocarditis is a cardiac manifestation of drug-induced delayed-type hypersensitivity.
  • Its incidence is notably higher in patients awaiting heart transplantation.

Observation:

  • A patient with end-stage heart failure received a left ventricular assist device (LVAD) as a bridge to transplantation.
  • Histologic examination of the left ventricular specimen during LVAD implantation revealed hypersensitivity myocarditis.

Findings:

  • The hypersensitivity myocarditis lesion demonstrated complete reversibility.
  • Reversibility was observed in myocardial tissue explanted ten months after LVAD implantation.

Implications:

  • Cessation of inotropic therapy following LVAD implantation may contribute to the resolution of hypersensitivity myocarditis.
