Summary
Aneurysmal bone cysts (ABCs) are rare in the skull. This case highlights the diagnosis and successful surgical removal of a temporal bone ABC in a young patient.

Background:
- Aneurysmal bone cysts (ABCs) are uncommon bone lesions.
- ABCs rarely occur in the temporal bone and calvarium.
Observation:
- A 14-year-old boy presented with swelling in the right temporal region.
- MRI revealed a destructive, expansile bone lesion in the right anterior temporal and orbital bone.
Findings:
- The lesion was completely removed via right temporal craniotomy and orbitozygomatic osteotomy.
- Pathologic examination confirmed the diagnosis of an aneurysmal bone cyst.
Implications:
- This case underscores the rarity of temporal bone ABCs.
- Complete surgical excision is the preferred treatment for resectable ABCs.
