Summary

Distinguishing severe hyperreactio luteinalis from spontaneous ovarian hyperstimulation in early pregnancy can be challenging. Both conditions present with bilateral ovarian enlargement during the first trimester.

Two cases of bilateral ovarian enlargement in the first trimester of pregnancy are presented.
One patient was diagnosed with spontaneous ovarian hyperstimulation syndrome (OHSS).
The second patient was diagnosed with hyperreactio luteinalis (HRL).

Findings:

Clinical differentiation between severe HRL and OHSS can be difficult.
Both conditions manifest similarly in the first trimester.

Implications:

This highlights the importance of careful clinical evaluation and diagnostic considerations.
