Summary

Postmenopausal bleeding can stem from chronic disseminated intravascular coagulation (DIC) due to aortic aneurysm. This rare cause was successfully managed with blood products and tranexamic acid.

Area of Science:

  • Vascular Surgery
  • Hematology
  • Gynecology

Background:

  • Postmenopausal bleeding (PMB) necessitates thorough investigation to identify underlying causes.
  • Bleeding disorders are an infrequently cited cause of PMB, with limited documented cases.
  • Aortic aneurysms are rarely associated with systemic bleeding complications like disseminated intravascular coagulation (DIC).

Observation:

  • A patient presented with significant postmenopausal bleeding.
  • The bleeding was attributed to chronic disseminated intravascular coagulation (DIC).
  • The underlying cause of DIC was identified as a ruptured aortic aneurysm.

Findings:

  • This case highlights a rare etiology of postmenopausal bleeding: chronic DIC secondary to aortic aneurysm.

Related Experiment Videos

  • The patient's bleeding was effectively managed using a combination of blood product replacement and tranexamic acid.
  • Diagnostic workup included imaging to identify the aortic aneurysm and coagulation studies to confirm DIC.
  • Implications:

    • Clinicians should consider rare causes of postmenopausal bleeding, including DIC secondary to vascular pathology.
    • Prompt diagnosis and management of aortic aneurysms are crucial in patients with unexplained bleeding.
    • This case expands the understanding of the clinical manifestations of aortic aneurysms and DIC.
