Summary

This case report describes a rare tumefactive lipedema presentation in a 60-year-old woman. The findings suggest a potential link between lipedema and pseudoxanthoma elasticum (PXE).

Area of Science:

  • Dermatology
  • Genetics
  • Pathology

Background:

  • Lipedema is a chronic condition causing painful, symmetrical swelling in the legs and buttocks.
  • Microscopic features include edema, adipocyte degeneration, and mast cell infiltration.
  • Lipedematous alopecia shares some microscopic features with lipedema.

Observation:

  • A 60-year-old woman presented with bilateral leg masses and microscopic lipedema features.
  • Elastic-fiber changes, typical of pseudoxanthoma elasticum (PXE), were found in subcutaneous septa.
  • The patient had a history of hypertension and congestive heart failure but no other clinical PXE signs.

Findings:

  • This represents a previously undescribed tumefactive presentation of lipedema.
  • Elastic-tissue abnormalities suggest either subclinical PXE or secondary changes due to edema.

Related Experiment Videos

  • The findings may indicate an unusual manifestation of localized acquired cutaneous PXE (calcific elastosis).
  • Implications:

    • This case expands the understanding of lipedema's varied presentations.
    • It raises questions about potential genetic predispositions or secondary changes in lipedema.
    • Further research is needed to clarify the relationship between lipedema and PXE.
