Area of Science:

  • Oncology
  • Urologic Oncology
  • Cancer Biology

Background:

  • Muscle invasive bladder cancer (MIBC) is a heterogeneous disease.
  • Chemotherapy offers improved outcomes for some MIBC patients.
  • Current staging lacks criteria for optimal patient selection for chemotherapy.

Purpose of the Study:

  • To advocate for a biologically-based staging system for MIBC.
  • To emphasize the need for improved patient selection for perioperative chemotherapy.

Main Methods:

  • Review of current MIBC treatment paradigms.
  • Analysis of MIBC heterogeneity.
  • Discussion of biologically-based versus anatomical staging.

Main Results:

Related Experiment Videos

  • Chemotherapy is beneficial in MIBC but patient selection remains a challenge.
  • Anatomical staging is insufficient for guiding chemotherapy decisions.
  • A biologically-based staging system is proposed as a solution.
  • Conclusions:

    • A biologically-based staging system is crucial for optimizing MIBC treatment.
    • Perioperative chemotherapy with standard combinations should be considered with curative intent.
    • Further research into MIBC biomarkers is warranted for improved patient stratification.
