Summary

Sequential bilateral epidural hematomas are rare. This case highlights the importance of vigilant post-operative monitoring and repeat CT scans for patients with epidural hematomas who do not improve.

Area of Science:

  • Neurosurgery
  • Neurology
  • Radiology

Background:

  • Bilateral extradural hematomas are infrequently documented.
  • Sequential development of extradural hematomas, occurring one after the other, is exceptionally rare.

Purpose of the Study:

  • To report a rare case of sequentially developed bilateral extradural hematoma.
  • To emphasize the importance of post-operative monitoring in epidural hematoma cases.

Main Methods:

  • Case report of a single patient.
  • Review of 187 operated epidural hematoma cases over 4 years.
  • Utilized emergency brain CT scans for diagnosis and monitoring.

Main Results:

  • A single case of sequentially developed bilateral extradural hematoma was identified.

Related Experiment Videos

  • Initial right temporoparietal epidural hematoma was evacuated.
  • Contralateral epidural hematoma developed, requiring reoperation, followed by complete recovery.
  • Conclusions:

    • Highlights the necessity of continuous patient monitoring post-epidural hematoma surgery.
    • Recommends repeat brain CT scans if clinical improvement is not rapid, especially within 6 hours of trauma.
    • Suggests intraoperative brain swelling as a potential indicator for contralateral hematoma development.
