Ocular tuberculosis presents with varied symptoms, sometimes mimicking eye tumors. Early diagnosis and multi-drug treatment are crucial, especially for immigrants, to manage this re-emerging public health concern.

Area of Science:

  • Ophthalmology
  • Infectious Diseases
  • Public Health

Background:

  • Tuberculosis (TB) is a growing global health issue.
  • Ocular manifestations of TB are rare but diverse.
  • TB can initially present with eye symptoms mimicking malignancy or inflammation.

Observation:

  • Five patients with ocular TB were evaluated, suspected of having ocular malignancy.
  • Four patients were recent immigrants to the United States.
  • Presenting features included panophthalmitis, endophthalmitis with scleral perforation, choroidal granuloma with uveitis, and amelanotic choroidal lesions.

Findings:

  • Two patients had a known history of systemic TB; three presented with ocular findings as the first sign of TB.
  • All patients received multi-drug anti-TB therapy for a mean of 9 months.

Related Experiment Videos

  • Three patients had successful globe salvage; two required enucleation.
  • Implications:

    • Ocular TB can present atypically, resembling intraocular or epibulbar tumors.
    • High clinical suspicion is vital for prompt diagnosis and management.
    • Particular vigilance is needed in immigrant populations from TB-endemic regions.
