Summary

Subacute sclerosing panencephalitis (SSPE) rarely affects the brainstem, but this case showed significant pontine involvement. Diffusion MRI revealed distinct patterns of affected and spared nerve fibers in the pons.

Background:

  • Subacute sclerosing panencephalitis (SSPE) is a rare, progressive neurological disorder.
  • Brainstem involvement in SSPE is uncommon, making this case unique.

Observation:

  • An 11-year-old boy presented with SSPE and prominent pontine involvement.
  • Diffusion magnetic resonance imaging (dMRI) was utilized to assess nerve fiber integrity.

Findings:

  • dMRI revealed high signal intensity in affected pontine regions on b = 1000 s/mm2 images.
  • Low apparent diffusion coefficient (ADC) values (0.43–0.54 x 10(-3) mm2/s) indicated restricted diffusion in affected areas.
  • The ventral pons, pontine tegmentum, and longitudinal pontine nerve bundles were spared, showing normal ADC values (0.89–1.04 x 10(-3) mm2/s).
