Area of Science:

  • Gastroenterology
  • Pulmonology
  • Oncology

Background:

  • Adenocarcinoma of the stomach presents with increased frequency and earlier onset in patients with chronic venous hypertension (CVH).
  • Lymphangitic carcinomatosis is a rare presentation of metastatic cancer.
  • Interstitial lung disease (ILD) can be caused by various conditions, including malignancy.

Observation:

  • A case report of a patient with CVH presenting with symptoms suggestive of interstitial lung disease.
  • The patient was diagnosed with lymphangitic carcinomatosis originating from adenocarcinoma of the stomach.

Findings:

  • This is the first reported case of lymphangitic carcinomatosis from gastric adenocarcinoma presenting as ILD in a CVH patient.
  • The findings highlight a critical diagnostic consideration in the evaluation of CVH patients with respiratory symptoms.

Related Experiment Videos

Implications:

  • Clinicians should maintain a high index of suspicion for gastric adenocarcinoma in CVH patients with ILD-like presentations.
  • Regular screening for gastric cancer, including stool occult blood tests and endoscopic evaluation for those with predisposing conditions (pernicious anemia, atrophic gastritis, achlorhydria), is recommended for CVH patients.
