Background:

  • High-resolution CT (HRCT) is crucial for diagnosing diffuse cystic lung diseases (DCLDs).

  • Current pulmonary function tests have limited sensitivity and reliability for assessing lung impairment in DCLDs.

Purpose of the Study:

  • To correlate quantitative HRCT density mask (DM) scores with pulmonary function data in DCLDs.
  • To assess the utility of DM software in evaluating lung impairment.

Main Methods:

  • HRCT with DM quantitative evaluation was performed on 25 DCLD patients (13 lymphangioleiomyomatosis, 12 Langerhans cell histiocytosis).
  • Pulmonary function tests included spirometry, lung volumes, diffusion capacity, arterial blood gas analysis, and the 6-minute walking test (6-MWT).

    • An obstructive pattern was observed in 52% of patients, and a restrictive pattern in 12%.

    • No significant correlation was found with arterial blood gas analysis results.

    • HRCT with DM quantitative assessment strongly correlates with functional parameters in DCLDs.
    • DM software can be a valuable tool in the initial evaluation of DCLD patients, alongside complete functional assessments.
    • Further research is needed to determine its utility in monitoring disease progression.
