Background:

  • Myocardial injury can disrupt intraventricular conduction, altering the QRS complex.

Purpose of the Study:

  • To identify microvolt early waves at the QRS onset in post-infarction patients with reduced ejection fraction.
  • To investigate the association between these early waves and ventricular arrhythmias.

Main Methods:

  • High-resolution ECG recordings utilizing a "pyramid" electrode placement.
  • Analysis of high-frequency potentials, QRS duration, ejection fraction, and ventricular tachycardia/fibrillation occurrence.

  • Early waves were more prevalent in secondary VT/VF (30%) than primary VT/VF (8%) and absent in patients without MI or VT/VF.

Conclusions:

  • The presence of early potentials, late activity, and prolonged ventricular activation time signifies myocardial electrical instability and reduced ejection fraction.
  • Early waves serve as a critical diagnostic marker for poor hemodynamic status and predict recurrent ventricular tachycardia/fibrillation.
