Background:

  • Diabetic macular edema (DME) is a leading cause of visual impairment in working-age adults.
  • The underlying mechanisms of DME pathogenesis remain incompletely understood.
  • Current treatments for DME may not be effective for all patients.

Purpose of the Study:

  • To investigate the role of retinal hypoxia in the development and progression of diabetic macular edema.
  • To assess the impact of supplemental oxygen therapy on retinal thickness and visual acuity in patients with DME.

Main Methods:

  • A small cohort of five patients (nine eyes) with chronic DME received 4 L/min of inspired oxygen for three months.
  • Retinal thickness was measured using optical coherence tomography (OCT).

  • Best corrected visual acuity (VA) was assessed at baseline, during oxygen therapy, and post-therapy.
  • Main Results:

    • All nine eyes showed a significant reduction in macular thickness after three months of oxygen therapy.
    • Average reductions were 43.5% in foveal thickness and 54% in macular volume.
    • Three eyes demonstrated visual acuity improvement of at least two lines; however, thickening recurred in five eyes after oxygen discontinuation.

    Conclusions:

    • Supplemental inspired oxygen can decrease macular thickness in DME patients.
    • These findings suggest that retinal hypoxia is a contributing factor in DME.
    • Further research into oxygen therapy for DME is warranted.
