Background:

  • Temporal lobe epilepsy (TLE) is associated with various psychiatric and behavioral changes, including mood disorders and altered sexual behavior, both before and after surgical intervention.
  • Selective amygdalohippocampectomy is a surgical procedure for medically intractable TLE, particularly with hippocampal sclerosis.

Observation:

  • A 14-year-old female patient with medically intractable TLE underwent selective amygdalohippocampectomy.
  • Post-surgery, she exhibited new-onset behavioral issues: excessive masturbation in public, social withdrawal, irritability, aggression, and crying spells.
  • Family distress due to the patient's behavior exacerbated her depressive symptoms.

Findings:

  • The patient's hypersexual behavior (excessive masturbation) and depressive symptoms significantly improved within two months.

  • Post-surgical hypersexuality is hypothesized to stem from psychosocial adjustments to seizure freedom, Kluver-Bucy syndrome features, or altered neurophysiology post-epileptic discharge cessation.

    • In adolescents, hypersexuality post-TLE surgery may present as excessive masturbation.
