  • Increasing use of thoracic pedicle screws necessitates better understanding of scoliotic pedicle anatomy.
  • Limited studies have evaluated the morphology of thoracic pedicles in scoliotic spines.

Purpose of the Study:

  • To quantify morphological changes in thoracic pedicles affected by scoliotic deformity.
  • To compare pedicle dimensions and orientation between scoliotic and normal thoracic vertebrae.

Main Methods:

  • Morphometric analysis of 325 scoliotic and 358 normal thoracic vertebrae.
  • Three-dimensional reconstruction and digitization of vertebrae.
  • Calculation of 22 pedicle parameters, including width, length, height, surface, and orientation.

Main Results:

  • Pedicles on the concavity of thoracic curves were significantly thinner (up to 1.68 mm difference).
  • Pedicle length was slightly increased, and height slightly decreased in scoliotic spines.
  • Pedicle orientation and inclination remained unchanged relative to the vertebral body.

Conclusions:

  • Pedicle width is significantly reduced on the concave side of thoracic scoliotic curves.
  • Caution is advised when using thoracic pedicle screws, particularly on the concave side.
  • Findings are critical for spinal corrective surgery in adolescent idiopathic scoliosis (AIS).
