Background:

Left atrial appendage aneurysm is a rare congenital anomaly.
Diagnosis in pediatric patients is uncommon, often presenting incidentally.
Transthoracic echocardiography is a key diagnostic tool.

Observation:

A 1-year-old child presented with an incidentally discovered mediastinal liquid mass.
Transthoracic echocardiography confirmed the mass as a left atrial appendage aneurysm.
The patient was asymptomatic prior to diagnosis.

Findings:

The left atrial appendage aneurysm was successfully diagnosed and surgically excised.
Aneurysmectomy was performed using cardiopulmonary bypass and cardioplegic arrest.
Histopathological confirmation of the aneurysm was obtained.
