Summary
Wegener's granulomatosis can present with diffuse pulmonary hemorrhage, a rare and life-threatening complication. This case highlights the rapid deterioration and fatal outcome associated with this unusual manifestation.
Area of Science:
- Pulmonology
- Rheumatology
- Pathology
Background:
- Wegener's granulomatosis (WG) is a rare autoimmune disease characterized by granulomatous inflammation and vasculitis.
- Pulmonary involvement is common in WG, but diffuse alveolar hemorrhage (DAH) is an uncommon and severe manifestation.
Observation:
- A 58-year-old male presented with hemoptysis and fever, later diagnosed with WG via transbronchial lung biopsy.
- Despite immunosuppressive therapy (cyclophosphamide and prednisolone), the patient experienced rapid clinical deterioration with diffuse alveolar shadows on chest X-ray.
Findings:
- Autopsy confirmed diffuse alveolar hemorrhage with necrotizing capillaritis, characterized by neutrophilic infiltration and fibrin thrombi, alongside typical WG pathology.
- Necrotizing capillaritis in the lungs was identified as the cause of DAH in this WG case.
Implications:
- Diffuse pulmonary hemorrhage in WG is a critical, life-threatening complication requiring prompt diagnosis and aggressive management.
- This case underscores the importance of recognizing DAH as a potential complication of WG for timely intervention and improved patient outcomes.
