    Background:

    • Severe mitral regurgitation (MR) can present atypically.
    • Understanding the relationship between left atrial size, pulmonary pressures, and MR is crucial.

    Observation:

    • The syndrome of severe MR with a normal-sized left atrium may present without pulmonary hypertension or prominent left atrial V waves.
    • Explaining these pressure alterations solely by a single pressure-volume relationship may be insufficient.

    Findings:

    • Normal pulmonary vascular pressures do not exclude the diagnosis of severe mitral regurgitation.
    • The presentation of severe MR is more complex than previously thought.

    Implications:

    • Clinicians should consider mitral regurgitation even when pulmonary pressures appear normal.
    • Further research is needed to elucidate the mechanisms behind atypical MR presentations.
    • Diagnostic approaches for mitral regurgitation may require revision.
