Summary

Combined medical abortion regimens are more effective than single agents for first trimester pregnancy termination. Vaginal misoprostol is more effective than oral, and mifepristone dosage can be reduced without compromising effectiveness.

Area of Science:

  • Reproductive Health
  • Pharmacology
  • Evidence-Based Medicine

Background:

  • Medical abortion emerged as an alternative to surgical abortion in the first trimester.
  • Key drugs include prostaglandins, mifepristone, and methotrexate, used alone or in combination.

Purpose of the Study:

  • To compare the effectiveness and safety of various medical methods for first trimester abortion.

Main Methods:

  • Systematic review of randomized controlled trials from Cochrane Controlled Trials Register, MEDLINE, and Popline.
  • Inclusion criteria focused on trials with adequate methodology and first-trimester participants undergoing medical abortion.
  • Outcomes assessed included abortion success, need for surgical intervention, and side effects.

Main Results:

Related Experiment Videos

  • Combined mifepristone/prostaglandin regimens are more effective than single agents.
  • Vaginal misoprostol is more effective than oral administration.
  • Lower mifepristone doses (200 mg) are as effective as higher doses (600 mg) in combined regimens.

Conclusions:

  • Safe and effective medical abortion options exist, with combined regimens showing superior efficacy.
  • Vaginal misoprostol administration and reduced mifepristone dosage offer effective options.
  • Applicability of findings to under-resourced settings requires further investigation due to trial settings.
