Summary

Low-dose helical CT with 8 mm collimation significantly improves visualization of segmental bronchi for early lung cancer screening. A sequential subsection scan technique is recommended for optimal detection.

Area of Science:

  • Radiology
  • Pulmonology
  • Oncology

Background:

  • Early detection of central lung cancer is crucial for improving patient outcomes.
  • Low-dose helical computed tomography (CT) is a promising tool for lung cancer screening.
  • Optimizing CT scanning parameters, such as collimation, is essential for enhancing diagnostic accuracy.

Purpose of the Study:

  • To evaluate the impact of varying scanning collimations in low-dose helical CT on the visualization of segmental bronchi.
  • To determine the optimal scanning collimation for effective CT screening of early central lung cancer.

Main Methods:

  • Forty healthy volunteers underwent low-dose helical CT scans (120 kVp, 50 mA, 0.75 s, pitch 1).
  • Participants were divided into two groups: one received a sequential subsection scan with variable collimation (10 mm, 8 mm, 5 mm), and the other received a whole-lung scan with 10 mm collimation.

Related Experiment Videos

  • Visualization of lobar and segmental bronchi was compared across different collimations using ANOVA and multiple comparison tests.
  • Main Results:

    • Segmental bronchi visualization was significantly higher with 8 mm (88%) and 5 mm (91%) collimation compared to 10 mm (60%) collimation (P < 0.01).
    • The right and left upper lobe segmental bronchi were well-visualized across all tested collimations (82-98%).
    • Visualization of the lingular lobe segmental bronchi was challenging, with improvement from 30% at 10 mm to 70% at 5 mm collimation.

    Conclusions:

    • Low-dose helical CT with 8 mm collimation reliably visualizes lobar and segmental bronchi.
    • A sequential subsection scan of the chest, divided into three zones, is an effective approach for CT screening of early lung cancer.
