Summary

Venogram-guided extrathoracic subclavian vein puncture is a safe and effective method for permanent pacemaker lead insertion. This technique offers a successful approach with no major complications like pneumothorax or infection.

Background:

Subclavian vein puncture is a standard procedure for permanent pacemaker lead placement.
Assessing alternative methods for lead insertion is crucial for patient safety and procedural success.

Purpose of the Study:

To evaluate the safety and feasibility of venogram-guided extrathoracic subclavian vein puncture.
To determine the efficacy of this technique for permanent pacemaker lead insertion.

Main Methods:

A retrospective analysis of 60 patients undergoing permanent pacemaker implantation.
Extrathoracic subclavian vein puncture guided by venography was employed for lead insertion.
Procedures were performed between March 2002 and December 2002.

The procedure was successful in all 60 patients.
Two patients experienced inadvertent subclavian artery puncture without complications.
No cases of pneumothorax, hemothorax, or pacemaker site infection were reported.

Conclusions:

This approach can be considered a preferred method for permanent pacemaker lead insertion.
