Central hepatectomy (CH), a major liver resection, is suitable for centrally located lesions. This study found CH procedures for colorectal metastases and inflammatory pseudotumor had good outcomes with minor complications.

Area of Science:

  • Hepatobiliary surgery
  • Surgical oncology
  • Gastroenterology

Background:

  • Central hepatectomy (CH) involves resecting Couinaud's segments IV, V, and VIII.
  • It is indicated for centrally located liver lesions, aiming to preserve functional parenchyma and prevent liver failure.

Observation:

  • A review of 507 liver resections performed between January 1995 and November 2002 identified three cases of CH.
  • These CH procedures were performed for one case of colorectal metastases and two cases of inflammatory pseudotumor.

Findings:

  • The mean operative time for CH was 231 minutes, with an average blood loss of 915 ml.
  • Patients undergoing CH experienced a favorable postoperative course with only minor complications.

Implications:
